We do NOT work with Medicaid dental policies, which includes Caresource, Molina, Humana Healthy Horizons, Buckeye Community Health Plan, and UnitedHealthCare Community Health Plan.

The listed companies have a large variety of individual plans. Thus, plans under the companies listed are not guaranteed to be in-network. Please contact our office to verify network status with your particular plan.

All patients are welcome regardless of insurance status, and we encourage you to consider our affordable Membership Plan. 

*Patients Cigna DPPO and Cigna DPPO Advantage, please call us to verify in/out-of-network status as there are many variations of these plans. We are not accepting Cigna DHMO plans.

**We are in-network with Metlife PDP Plus and FEDVIP. We are accepting patients with Metlife PDP as an out-of-network provider. The PDP plan offers great coverage for out-of-network dental visits, we have many patients with Metlife PDP dental insurance who choose to use their coverage with us, and we are happy to answer any questions. This arrangement allows us to continue providing the best quality of care for you.

***We are in-network with United Concordia Elite Plus and Alliance plans. We are accepting patients with United Concordia Advantage plans as an out-of-network provider. The Advantage plan offers great coverage for out-of-network dental visits, we have many patients with United Concordia Advantage dental insurance who choose to use their coverage with us, and we are happy to answer any questions. This arrangement allows us to continue providing the best quality of care for you.

**** We are temporarily out-of-network Aetna beginning September 11th, 2023 due to contract restructuring. We plan to restore in-network status as soon as possible. During the transition period, we will be honoring in-network fees for Aetna to make sure you are taken care of through this process. Please call our office if you have any questions.

*****At this time, we are not scheduling new patients wishing to use their Liberty dental plans with us. Patients of record with Liberty can be seen as usual. We will be done with contract negotiations at the end of summer, so new patients please check back in late July/early August. Thank you for your patience.
